Subject: Hardship letter for social security

Respected Mr. Carter,

This letter is to bring to your notice that I am going through a hardship as I do not have a social security number. I need a social security number urgently and would request you to please process it.

I had undergone a medical test last week and I have been diagnosed with terminal illness for which I need to undergo treatment as soon as possible. If there is delay in undergoing treatment the disease may reach its critical stage. I had a word with the hospital authorities about the treatment cost and it would be impossible for me to afford the medical expenses. In order to meet the medical expenses I have to avail government aid and it is necessary for an individual to have a social security number in order to avail government aid.

I am enclosing all the necessary documents along with the letter. I request you to please consider my hardship and process my social security number as soon as possible.

Regards,

Marie Ann Cooper
